One to One (North West)Ltd. Has taken over three years to get to this point. The work of a dedicated passionate midwife who believes in every woman’s right to a safe satisfying birthing memory. The clinical director and founder of OTO has been a midwife for many years both in the NHS and as an Independent midwife, she is support by three senior midwives all with substantial NHS experience. Over the past twenty five years or more the NHS have tried and failed to implement a model of continuity of care. We at OTO can do what the NHS cannot, one women, one midwife, one relationship. The ethos of One to One is that the women and her family are at the very centre of care.
We aim to offer a service that will impact the long term public health of women and their families. As I am sure you are aware we in the health service have been fighting against the inequalities in health as highlighted in the Black report in 1980 and little has changed. Through the One to One caseloading model of care midwives develop a unique relationship with the women and her family. One to One philosophy of care works from the premise that birth is a normal life event within the context of a social model of health and wellness. This perspective includes the wider determinants of health, social, psychological issues and emotional support for women and their families, working across boundaries with different agencies at each birth setting, including home birth.
The OTO midwives aim to connect with women pre-conceptually or by six weeks pregnant. Women are referred to the OTO program through their GP or more frequently by self referral, facebook, One to One website or twitter, following that referral the woman will have face to face contact with a named midwife within 48 hours, building a relationship that will last until the baby is six weeks old. One to One midwives are not constricted by NHS inflexible antenatal and post natal visiting regimes and are therefore able to offer a flexible model of care wherever and whenever the woman wishes. In addition, while OTO adhere to national antenatal visiting schedule as set out in NICE guidelines we to do not limit antenatal or postnatal visits, the midwife will individualise care and conduct as many visit as necessary for optimal care outcomes. Women booked with OTO will have 100% of routine visits conducted by their named midwife and 70% of women will know the midwife that helps them birth their baby. In addition, women defined as having a medical or obstetric risk in pregnancy can also book with our service. They will have a named midwife who will work in partnership with the women and her obstetrician, advocating, supporting, navigating her care pathway to empower her, ensuring that she is part of the decision making process, achieving a happy birth memory.
One to One acknowledges that midwives play a vital role in health promotion of women and their families. It is at the early stage of pregnancy or pre-conception that the foundation for healthy lifestyle or lifestyle changes can be made and through support sustained throughout the pregnancy and beyond. One to One model enables midwives to provide an enhanced service provision, which will meet the needs of women and their families. One to One midwives will identify problems early and focus on identifying issues around mental health and lifestyle choices. Early intervention in these areas have shown improved health outcomes due to an increase in women’s motivation and behaviour change that is achieved with consistent and continuous support. This service model has been shown to improve access to services and impacts on health inequalities, particularly on Public Service Agreement (PSA) targets of reduction in infant mortality rates and increase in breastfeeding rates, and tackling the rise in obesity.
The evidence shows that the partnership interaction and trusting relationship between the midwife and her client empowers women to informed decision making which in turn Increases satisfaction, Increases optimal birthing outcomes, reduces risk, decreases instrumental/surgical intervention, reduced caesarean section rates, increases the normal, home birth and breast feeding rates which in turn Impacts on long term health outcomes
Our goal is to change the focus of maternity services from a medical to a social model of care and in so doing restore our midwifery profession
We are currently working across the Wirral but believe that every woman has a right to this type of care and so aim to roll out this service across the country.
